Patient Rights and Responsibilities

As a patient, you have certain rights and responsibilities concerning your care and
treatment. These rights and responsibilities exist to ensure that we provide you with
important information about your care and that you receive treatment in a safe and
courteous manner.

Patient Rights

As a patient you have the right to...

  • Be treated with courtesy, respect and appreciation of your individual cultural
    or religious beliefs.
  • Personal dignity
  • Receive information and be included in decisions involving your care.
  • Impartial access for medical treatment or accommodations, regardless of race, national origin, religion, physical handicap or source of payment.
  • Treatment for any emergency medical condition that will deteriorate if treatment is not provided.
  • Know the name of the person who is providing medical services and who is responsible for your care.
  • Have your doctor and a family member notified when you are admitted.
  • Receive information about your diagnosis, health status, planned course of treatment, alternatives, risks and prognosis from your doctor.
  • Assessment and management of your pain.
  • Request and refuse any treatment, except as otherwise provided by law.
  • Make an Advance Directive and expect that your doctor and hospital staff provide care that is consistent with these Directives.
  • A prompt and reasonable response to questions and requests.
  • Be free from both physical and chemical restraints except when necessary and less restrictive interventions have been determeined to be ineffective.
  • Be free from abuse or harassment in any form.
  • The protection of your need for privacy and to receive care in a safe setting.
  • Confidentiality of your medical record.
  • Access to information contained within your medical record within a reasonable time frame.
  • Know what rules and regulations apply to your conduct.
  • Know what patient support services are available, including whether an interpreter is available if you do not speak English
  • Know if medical treatment is for purposes of experimental research and to give your consent or refusal to participate in such experimental research.
  • Be given, upon request, full information and necessary counseling on the availability of known financial resources for your care.
  • If you are eligible for Medicare, to know upon inquiry and in advance of treatment, wheather the health care provider or health care facility accepts the Medicare assignment rate.
  • Receive, upon requrest, a reasonable estimate of charges for medical care.
  • Receive a copy of a reasonably clear and understandable itemized bill, and
    upon request, have charges fully explained.
  • Express concerns or grievances regarding any violation of your rights or any other issue to hospital administration. If your concern or grievance cannot be resolved, you have the right to further express your grievance to the home health administrator. (address and telephone numbers are available upon request).

Patient Responsibilities

As a patient you are responsible to...

  • Give accurate and complete information about your present complaint, past illnesses, prior hospitalizations and any medications you are taking.
  • Be involved in the decisions that affect your care.
  • Ask questions and “Speak Up” if you have any questions or concerns about your care, treatment or safety.
  • Follow the treatment plan recommended by your healthcare provider. You take responsibility for your decisions if you refuse treatment or do not follow your physician’s instructions.
  • Tell your doctor or other healthcare member if you do not understand your treatment plan or treatment options.
  • Report any unexpected changes in your condition to your healthcare team.
  • Keep appointments. If for any reason you cannot keep an appointment, notify the healthcare provider.
  • Assure that any financial obligations concerning your healthcare are fulfilled as promptly as possible.
  • Follow the heathcare facility’s rules and regulations affecting patient care and conduct.
  • Be respectful to other patients and recognize their right to privacy.

Concerns or Complaints
Your satisfaction is important to us. If you have a concern or a complaint, please allow the person responsible for your care or their supervisor the opportunity to listen, review and to assist with an appropriate resolution. If your concern or complaint is unresolved, please contact administration and ask to speak with the administrator on call.

Ethics Committee
Home health has an Ethics Committee available to assist you with difficult decisions concerning care or treatment. If you would like to meet with the Ethics Committee, please ask your nurse.
